Hafiz obtained his PhD from the Joint Physics Department, Institute of Cancer Research London and the Royal Marsden Hospital, UK. During his PhD, he was involved in developing prototype CMOS image sensors for real-time radiotherapy treatment verification, as part of a multi-institutional and multi-disciplinary research consortium funded by the Engineering and Physical Sciences Research Council (EPSRC UK). In 2012, he returned to Malaysia to join Universiti Sains Malaysia as a lecturer, and was appointed Associate Professor in 2023. He is currently Head of the Biomedical Imaging Department at the university’s Tun Abdullah Ahmad Badawi Cancer Centre, since 1 February 2024, and also contributes to the Oncology & Radiotherapy Unit of the university’s hospital. His primary clinical and research focus is improving targeted radiotherapy treatment of cancer using optimised image guidance modalities and real-time treatment delivery tracking. He is Editor-in-Chief of the Physics and Technology in Medicine Journal, and the current President of the Malaysian Association of Medical Physics (MAMP). He has held various national and international appointments, including council member of the Malaysian Allied Health Professional Council under the Ministry of Health Malaysia (2020–2023), Fellow at the International Atomic Energy Agency, Vienna (2023), and visiting academic at Universitas Indonesia, Jakarta, Indonesia (2022–23).
